A primary goal of FACE is to support the healthy growth and development of FACE Home-based children.  Good health begins with children receiving early screening and interventions if needed.  Building trusting relationships with families and providing information and resources in a culturally competent manner makes a significance difference in how families receive and use health-related information.
Each FACE Program should follow the guidelines, policies and forms that clearly outline how children and adult screenings, re-screenings and referrals are carried out in FACE Home-based.

Goals of Screening:

Screening Tools:

  • Review with the parents/caregivers their child’s development process
  • Provide parents/caregivers with helpful information and guidance around child’s emerging skills
  • Identify possible problems or concerns in development and offer helpful interventions.
  • Decrease incidence of undetected development delays and physical concerns among school aged children
  • Increase the collaboration among FACE and community agencies serving families
  • ASQ-3 and ASQ:SE-2
  • Health Record
  • Milestones
  • OAE Hearing Screening
  • Eye, hearing and dental screenings offered through school or community agencies
  • Other tools as suggested and offered by PAT Home-based FACE
